Role Description Your North Star : Provide direct patient care and clinical oversight that optimizes outcomes through population-based implementation of evidence-based care. In this role you will: Be accountable for improving clinical outcomes for empaneled pediatric patients by overseeing their medical care Engage directly in patient care as a Pomelo provider providing care for acute issues as well as ongoing health education and care coordination Identify and utilize cross-functional team members to address patient needs Review complex patient cases, particularly for infants who are in or transitioning home from the NICU, develop care plans, and support other members of the clinical team in providing them with evidence-based care Monitor adverse events and hold clinical reviews to identify areas for improvement in Pomelo’s protocols Lead development and review of evidence-based medical protocols and algorithms related to pediatric care Actively participate in on-call schedules including overnight and on weekends as needed Requirements A minimum of 3 years of general pediatric experience as an advanced practice provider, in pediatric primary care or pediatric urgent care. Must have practiced in pediatric primary care or pediatric urgent care within the last 6 months. A passion for and demonstrated effectiveness in optimizing evidence-based care and outcomes Experience using data to drive patient engagement, activation, and outcomes Experience leading successful teams, with track record of outstanding collaboration and teamwork A sense of urgency to improve outcomes coupled with exceptional organization and attention to detail A growth mindset with the ability to approach process change and ambiguous situations with enthusiasm, creativity, and accountability Facility using multiple tech platforms, with an eagerness for advising about platform improvements and adapting to new systems Eager to thrive in a fast-paced, metric-driven environment Phenomenal interpersonal and communication skills Education and training Pediatric or Family Nurse Practitioner with substantial experience in pediatric primary and/or urgent care Active, unrestricted license to practice in TX, GA, TN, or VA preferred, and/or willingness to obtain licenses in all US states. Residence in a nursing compact state required. Bonus points for Telehealth and/or remote monitoring experience Experience in outpatient or home-based management care of higher-risk patients Experience working in a NICU or caring for NICU graduates Ability to provide medical care comfortably in Spanish Schedule Position includes Thursday overnight (9p-9a ET) and Saturday and Sunday days (9a-9p ET). Position is a 0.9 FTE and benefits eligible.
