The Role The Behavioral Health Care Manager is a direct support role addressing the mental health needs of individuals affected by neurodegenerative diseases. You will provide care coordination and behavioral health support to patients, caregivers, and their loved ones while implementing Synapticure’s Collaborative Care program. This role involves working closely with medical providers, patients, and partners to drive meaningful change and ensure exceptional mental health care for this community.
