Job Title: MEDICAL CODER
Job Description: The Virtual Medical Scribe / Clinical Assistant provides real-time documentation and administrative support to medical providers during patient encounters. This role ensures accurate and timely charting, supports clinical workflows, and assists in coordinating patient care. The position requires strong attention to detail, excellent documentation skills, and the ability to work efficiently in a fast-paced clinical environment. Responsibilities may evolve based on provider needs, patient volume, and operational priorities.
Schedule: Monday–Friday, 8:00 AM–5:00 PM PST
Responsibilities:
• Medical Documentation & Charting:
• Document patient encounters in real time including history, examination findings, and provider decision-making.
• Accurately record medication lists, treatment plans, and follow-up instructions.
• Ensure charts meet clinical documentation standards and support timely medical record completion.
• Assist providers in completing procedure and visit documentation promptly.
• Clinical Workflow Support:
• Prepare charts and review documentation prior to patient visits.
• Assist with processing physician orders for procedures, imaging, medications, and related services.
• Coordinate documentation for procedures and follow-up care.
• Enter data and maintain accurate records within the electronic health record (Epic).
• Administrative & Coordination Support:
• Process incoming patient documentation and requests via fax, mail, portal, or internal systems.
• Retrieve imaging and diagnostic reports from online portals as needed.
• Support communication regarding scheduling, follow-ups, and provider instructions.
• Assist with general administrative tasks to support clinical operations.
• Team & Operational Support:
• Collaborate with providers, clinical teams, and administrative staff.
• Adapt to changing workflow demands and patient volume needs.
• Provide cross-coverage for administrative and coordination tasks when necessary.
• Maintain professionalism, accuracy, and confidentiality in all patient-related work.
Additional Expectations:
• Maintain strict confidentiality and comply with HIPAA standards.
• Demonstrate strong attention to detail and reliability.
• Communicate clearly and professionally with internal teams.
• Responsibilities may expand based on operational needs.
Qualifications:
• Strong typing and real-time documentation skills.
• Comfortable with medical terminology and clinical documentation standards.
• Ability to multitask and work efficiently in a fast-paced clinical environment.
• Experience with electronic health records (Epic preferred).
• Strong written and verbal communication skills.
• Previous experience as a medical scribe, healthcare assistant, or clinical documentation specialist preferred.
• Ability to work independently.
