This hybrid role requires <strong data-start="763" data-end="821">regular face-to-face visits to patients in their homes</strong>, complemented by occasional office-based meetings and remote clinical administration. Staff may choose to attend the office more frequently if preferred.</p> <p data-start="979" data-end="1411">The role is ideal for a Band 6 or Band 7 therapist experienced in adult neurorehabilitation or looking to develop specialist skills. You will be part of the Neuro Early Supported Discharge (ESD) team, providing intensive neurorehabilitation to adults with stroke or other neurological conditions who are newly discharged from hospital or at risk of hospital admission. The team covers Southampton City and parts of West Hampshire.</p> <p data-start="1413" data-end="1703"><strong data-start="1413" data-end="1427">Important:</strong> Access to a car is required, as the role involves travel to patient homes across Southampton, the New Forest, Hamble, Andover, and Fordingbridge. Car insurance must cover business use. A Trust laptop, work phone, Smartcard, and System One access are essential for the role.</p> <hr data-start="1705" data-end="1708"> <h3 data-start="1710" data-end="1736">Key Responsibilities</h3> <p data-start="1737" data-end="1762">In this role, you will:</p> <ul data-start="1763" data-end="3064"> <li data-start="1763" data-end="1880"> <p data-start="1765" data-end="1880">Conduct <strong data-start="1773" data-end="1827">regular face-to-face assessments and interventions</strong> with clients in their homes or community settings.</p> </li> <li data-start="1881" data-end="2063"> <p data-start="1883" data-end="2063">Work as part of a multi-professional neurorehabilitation team, collaborating with acute rehabilitation units, community rehab teams, inpatient wards, and rapid response services.</p> </li> <li data-start="2064" data-end="2177"> <p data-start="2066" data-end="2177">Manage a caseload of clients in community or ward settings, including oversight of rehabilitation assistants.</p> </li> <li data-start="2178" data-end="2344"> <p data-start="2180" data-end="2344">Assess and treat clients with complex or chronic neurological conditions, determining both profession-specific and multidisciplinary rehabilitation interventions.</p> </li> <li data-start="2345" data-end="2464"> <p data-start="2347" data-end="2464">Plan and implement comprehensive programmes of intensive rehabilitation in hospital, clinic, or community settings.</p> </li> <li data-start="2465" data-end="2616"> <p data-start="2467" data-end="2616">Support team operations by managing caseloads during team leader absence, attending MDT meetings, scheduling work, and providing specialist advice.</p> </li> <li data-start="2617" data-end="2734"> <p data-start="2619" data-e ...
