The Registered Nurse (RN) with Home Health is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing and evaluating care of Home Health patients and their families in their home setting. The RN provides the best clinical practice in an environment of caring and healing while working autonomously in the home. Utilizing the nursing process, the RN delivers care in compliance with hospice/home health policies and procedures, legal and regulatory standards. The RN utilizes the nursing process by use of technology, therapeutic intervention, evidence-based practice, and coordination of care with other health team members.

Minimum Qualifications
Current RN License in the state of practice
Basic Life Support (BLS) certification for healthcare providers
Valid driver’s license with reliable transportation, acceptable driving record, and current auto insurance
Bachelor’s Degree in Nursing (BSN)
Effective Communication Skills – Ability to communicate clearly and professionally with patients, families, and healthcare teams
Experience: RNs with less than 12 months of experience as an RN must complete their BSN within five years of their start date.
Note:
Bachelor of Science in Nursing (BSN) is required as of the job description's last update on 4/27/2025. Employees hired or promoted prior to this date will be held to the minimum requirements that were in place at the time of their promotion or hire.
Preferred Qualifications:
Bachelor’s Degree in Nursing (BSN)
One year of RN experience in Acute Care, Home Health, or Hospice settings

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, and driving a vehicle.
Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
