More info at <a href="http://Akidolabs.com">Akidolabs.com</a>.</p></div><p><strong>The Opportunity </strong></p> <ul> <li> <p>The Community Health Worker (CHW) is responsible providing support to patients and families as part of Rhode Island Primary Care interdisciplinary care team within a primary care setting. CHWs will be assigned to work with their assigned primary care and pediatric practices to coordinate care for a patient’s social determinant of health (SDOH) needs. The RIPCPC Community health workers are an integral part of the care management team. In this role, assessment of patient bio-psychosocial strengths and challenges will be integral in supporting their healthcare needs and conditions. As a CHW you will coordinate with RIPCPC interdisciplinary care teams and community-based organizations to bridge the gap in services for our patients.</p> </li> </ul> <p><strong>What you’ll do </strong></p> <ul> <li>Complete initial assessments with the patient and/or family member to identify specific areas of patient strengths and barriers to care.</li> <li>Work to build a trusting relationship through continued rapport building techniques.</li> <li>Empower patients to manage their care through motivational interviewing and goal setting.</li> <li>Coordinate and collaborate with interdisciplinary care team members at RIPCPC, family members, and healthcare providers.</li> <li>Advocate for patients and families to ensure the patients needs are represented and supported by their healthcare team.</li> <li>Documentation in EPIC and native electronic medical record of physician practice.</li> <li>Assist with setting up social services through a community-based organization including: <ul> <li>Food assistance o Housing transitions</li> <li>State and federal applications, insurance, and benefits</li> <li>Long term care resources</li> <li>Child and family resources</li> </ul> </li> <li>Understanding social determinants of health (SDOH) and how they effect a patient’s health.</li> <li>Collaborate with Behavioral health Clinicians as needed for patient care and improving patient outcomes.</li> </ul> <p><strong>Who you are </strong></p> <ul> <li> <p><strong>Qualifications:</strong></p> <ul> <li>CHW State course completion.</li> <li>Bachelor’s degree in a health-related field, social work, health care administration.</li> <li>Background in managing populations with complex medical and behavioral needs.</li> <li>Experience in community health setting or public health.</li> <li>Proven knowledge of Medicaid population.</li> <li>Experience in documenting in an electronic health record.</li> <li>Ability to organize and manage a caseload and assist with patient navigation.</li> <li>Full COVID 19 Vaccination is required.</li> </ul> <p><strong>Skills and Training: ...
