This role involves working closely with individuals to support their recovery and independence following illness or hospital discharge. The successful candidate will play a key role in delivering short-term, goal-focused interventions that promote mobility, function, and overall well-being.</p> <p><strong>Key Responsibilities</strong></p> <p>In this role, you will assess patients’ physical abilities, develop personalised reablement plans, and deliver physiotherapy interventions aimed at restoring independence.
